Bilingual Case Manager, Community Care Programs,
Full- time – 35hrs/ week.Polish Australian Welfare Association (PAWA) Inc is a not-for-profit community organisation providing community care services to older Polish immigrants and other target community members.
PAWA is seeking to appoint a full-time bi-lingual Case Manager who will work in the Polish Community Aged Care Packages and will be responsible for case management and delivery of high quality services to frail aged people of Polish background in South-West, Inner West and Western Sydney.
ESSENTIAL QUALIFICATIONS, EXPERIENCE AND SKILLS:
* Qualifications in aged care, social sciences, social work or welfare related discipline
* Experience in working in aged care industry
* Understanding of the needs and issues facing frail aged, people with disability and their carers with special focus on needs and issues of people of CALD background
* Demonstrated experience in case management and needs assessment of older people and their carers in the community care programs
* Sound understanding of aged care system * Ability to support in-home support workers * Ability to work as part of the team * Sound knowledge of OH&S legislation, * Well developed interpersonal and communication skills * Capacity to work in harmony with the philosophy, objectives and principles of the organisation * Demonstrated computer literacy * Bi-lingual person of Polish background * Valid and current NSW driver’s license.
SALARY:
Social and Community Services (SACS) State Award – Grade 3, Year depending upon skills, knowledge and experience.
