Polish Community Council of Victoria is a non-profit organisation based in Melbourne that supports Polish community in Victoria since 1963. Our services are focused on clients who are the most disadvanteged because of their age, frailty, illness or disablity.
Community Aged Care Packages Program provided by PCCV has been specifically funded by the Department of Health and Aging to support frail older people of Polish background living in the Eastern and Southern suburbs of Melbourne.

Key selection criteria:
- Qualifications in Social Sciences, Welfare or Community Health - Experience in assessment, case management and service brokerage in a community setting (preferably in the area of aged care) - Knowledge of and ability to access a range of community resources and professional services for clients - Demonstrated ability to work effectively with members of Polish community - Excellent communication skills in English and Polish (both written and oral)

DUTY STATEMENT-CACP CASE MANAGER
Aim of the Community Aged Care Packages Program
Community Aged Care Packages are planned and coordinated packages of community care services to assist older people with complex care needs
The program involves the provision of Community Aged Care Packages (CACPs). These packages (of care) are generally available to frail older persons who have been assessed as eligible for personal level hostel care by an Aged Care Assessment Team (ACAT).Services are delivered to clients in their own homes and within the community. The program can provide assistance withdaily living and managing social and safety issues of eligible clients. The CACP provided by PCCV has been specifically funded to support frail older persons of Polish-background living in the eastern and southern suburbs of Melbourne.
Position Objectives
• to provide culturally and linguistically accessible packages of care to eligible clients; • plan, implement and monitor packages of care in consultation with clients, carers, service providers and other significant people in client’s life; • develop and maintain links with mainstream and ethno generic service providers; • ensure that clients are reciving high quality care , clients’rights and responsibilities are enforced;
Key Responsibilities
Relationship with Committee-Reporting and Accountability
Case Management:
• Carry out initial assessments of prospective clients,liasing as appropriate with client, carers,existing service providers,ACATs and other professionals • Liaise with ACATs and other referring agencies to ensure coordinated implementation of services to eligible clients. • Plan, coordinate and implement an agreed care package. • Initiate care planning meetings as required, and conduct regular case reviews. • Monitor care packages,reviewing and adjusting as required in consultation with client/carer and other relevant parties. • Liase with clients/carers and other relevant parties when alternate care arrangements are required
• Counsel clients,carers and advocates. • Liaise with the co-ordinator in the matching of staff and clients; • Meet weekly with the co-ordinator,CACP Programme, to exchange information, discuss client-staff matching, client progress reports and programme coordination. • Identify and report on service gaps within program and lack of access to needed services. • Manage vacancies/waiting list in consultation with program coordinator.
Administration
• Complete all documentation regarding assessment,care plans and service provision. • Maintain up-to date client data. • Maintain records of expenditure. • Monitor program expenditure and in consultation with the director, ensure budgetary constrains are adhered to. • Assist in the identification and engagemant of appropriate service providers. • Assist in compiling data about the range of services and hours of service deliery clients are reciving as required. • Assist in the development and maintenance of appropriate procedures and systems to provide effective and efficient administration to the program. • As and when necessary be involved with the interviewing and appoitment of CACP Program staff. • Assist in management and maintenance of physical, financial and human resources of the program.
Policy
• Be aware of and work within the CACP guidelins, CACP outcome standards, relevant funding and service agreement, PCCV policies and procedures and issues which relate to delivery of culturally appropriate services. • Assist in the development of relevant policies,procedures and guidelines. • Participate in the development and monitoring of Quality Assurance and Best Practices within the CACP.
Professional
• Maintain a contemporary level of professional knowledge and competence through networking and participation in appropriate meetings, courses and other professional activities.
Other duties
• Carry out other duties as directed.
Organisational accountability and extent of authority.
The position is accountable to the Executive Committee via the Director and is supervised and supported by the CACP co-ordinator.
As the Case Manager is responsible for client services, the position is to have input to policy and planning for the service through the co-ordinator.
The position is expected to make decisions and judgements about the psychological, social and physical wellbeing of the above group, develop care packages and implement client care plans according to professional standards.
The position is held accountable for the quality and effectivenes of assessment and case management and the development of care packages.
The position requires making judgements and decisions upon the needs of the frail aged in relation to meeting those needs in conjuction with other services.
This position has authority to:
• monitor and supervise CACP casual staff; • utilise funds within allocated budgets; • assist in the preparation of annual reports; • make recomendations on: - program development - CACP casual staff dismissal;
Key selection criteria
1. Qualifications in Social Sciences, Welfare or Community Health.
2. Experience in assessment, case management and service brokerage in a community setting.Preferably in the area of aged care.
3. Knowledge of, and ability to access a range of community resources and professional services for clients.
4. Demonstrated ability to work effectively with a members of Polish community.
5 Ability to work independently with a high degree of initiative..
6 Excellent communication skills both written and oral.The ability to prepare clear written reports, maintain files and other program documentation. The ability to represent the program at community forums.
7. An understanding of financial practices and ability to program manage within given budgetary parametres.
8. A commitment to offering quality services to clients. The ability to maintain respect and empathy for service users within a professional framework.
9. A current Victorian drivers’ license.
10. An ability to speak fluently Polish and English.
11. An understanding of the ageing process and its cultural impact.
Other Helpful Skills, Knowledge and experience
1. A knowledge of other CACP programs and providers eithin the Eastern and Southern region.
2. Computer literacy in database, word processing and the software The Caremanage.
